The Cellular Energy, Oxidative Damage, Oxalate & Mitochondrial Markers panel from World Health Laboratories is a highly specialized urine-based test designed to assess mitochondrial function, energy metabolism, oxidative stress, and oxalate burden. This advanced analysis offers critical insights into the biochemical processes that power the body’s energy production systems and reveal hidden dysfunctions at the cellular level.

What It Tests

This panel measures a wide range of organic acids that are involved in the Krebs cycle (citric acid cycle), glycolysis, and fatty acid metabolism. It also identifies markers for oxidative stress, genetic or acquired mitochondrial dysfunction, and oxalate overproduction or accumulation, which can have downstream effects on energy, cognition, pain, and chronic disease.

Key Metabolites Include:

  • Lactic acid & Pyruvic acid – Indicators of impaired aerobic metabolism or low oxygen states
  • Succinic, Fumaric, Malic, Isocitric, α-Ketoglutaric acids – Mitochondrial intermediates, often elevated with nutrient deficiencies (B-vitamins, CoQ10) or toxin exposure
  • Oxalic, Glycolic, Glyceric acids – Associated with genetic or microbial causes of oxalate toxicity
  • 3-Hydroxy-3-methylglutaric (HMG) – Reflects need for CoQ10 and mitochondrial biosynthesis status
Clinical Relevance

This test is essential for individuals presenting with:

  • Chronic fatigue and low energy
  • Neurodegenerative and neurocognitive symptoms
  • Mood disorders and anxiety
  • Fibromyalgia and unexplained pain syndromes
  • Cardiometabolic dysfunctions (e.g. insulin resistance, obesity)
  • Multiple chemical sensitivities or heavy metal exposure

It helps in identifying:

  • Functional nutrient insufficiencies (B-vitamins, CoQ10, glutathione)
  • Mitochondrial stress due to toxic exposures (e.g. lead, mercury, arsenic)
  • Enzyme blockages in the Krebs cycle and oxidative phosphorylation
  • Imbalances in oxalate metabolism linked to chronic inflammation and joint pain
Clinical Utility

This panel enables:

  • Early detection of mitochondrial inefficiencies before overt symptoms appear
  • Targeted therapeutic strategies: antioxidant support, nutrient repletion, detoxification support
  • Monitoring response to nutritional interventions or environmental exposure mitigation

Sample Type

  • Urine (First Morning Void Preferred) – Non-invasive and ideal for measuring dynamic metabolic byproducts
  • Includes: Evaluation of mitochondrial markers, oxidative damage indicators, and oxalate levels
  • Sample requirement: 20 ml midstream urine

Suitable for postal submission.
