The Metabolic & Cofactor Insight panel combines the Organic Acids Test with Vitamin Panel 16. Together, they evaluate functional metabolic output alongside directly measured vitamins, active coenzyme forms, antioxidants, carotenoids, CoQ10 and carnitine.
Why combine these two tests?
Organic acids are downstream products of cellular metabolism. Their patterns may suggest altered pathway flux, increased metabolic demand, microbial contribution, oxidative stress or reduced efficiency of nutrient-dependent enzymatic reactions. However, an organic-acid pattern alone does not establish that a specific vitamin is deficient.
Vitamin Panel 16 adds a complementary biochemical perspective by measuring circulating vitamins and related nutrient systems involved in these reactions. Combining functional metabolites with direct nutrient measurements helps the practitioner determine whether findings are concordant or whether other influences—such as altered utilisation, enzyme inhibition, increased turnover, medication effects, dietary substrate load or impaired absorption—should be considered.
The clinical advantage of the combined panel
Concordant findings—for example, a low directly measured nutrient together with a compatible functional metabolite pattern—may strengthen the case for clinically relevant insufficiency.
Discordant findings—such as an abnormal functional marker despite apparently adequate circulating nutrient status—may direct attention toward utilisation, transport, enzyme activity, oxidative demand, renal handling or other upstream influences rather than simple replacement alone.

Clinical and biochemical perspective
Krebs-cycle, glycolytic and fatty-acid oxidation metabolites can be interpreted alongside thiamine, riboflavin, niacin-related status, pantothenic acid, biotin, CoQ10 and carnitine. These nutrients support dehydrogenase activity, electron transfer, coenzyme A formation and mitochondrial substrate transport.
Functional markers such as methylmalonic acid, FIGLU and related metabolites may be considered together with measured folate, vitamin B12, riboflavin and vitamin B6 status. This supports a more differentiated assessment of nutrient availability versus altered cofactor utilisation or increased pathway demand.
Urinary metabolites related to tryptophan and catecholamine pathways can be reviewed with vitamin B6, riboflavin, folate, vitamin B12, vitamin C and other nutrient systems involved in synthesis, conversion and degradation. These findings remain peripheral metabolic indicators and should not be interpreted as direct measurements of brain neurotransmitter concentrations.
Organic-acid markers associated with oxidative or glutathione demand can be considered alongside vitamins C and E, carotenoids and CoQ10. This helps distinguish limited antioxidant availability from increased oxidative consumption, although it does not directly quantify oxidative tissue damage.
Direct nutrient measurements provide information about current biochemical status, while organic-acid patterns may reflect how nutrients are being used within metabolic pathways. Divergence between the two may warrant consideration of gastrointestinal absorption, dietary intake, supplementation, medication, renal function and inflammatory or metabolic demand.

Vitamin Panel 16: venous blood collected by a qualified blood-draw professional using the supplied collection kit.

Submission: suitable for postal submission when the supplied collection, storage and transport instructions are followed.
Preparation and collection
- Collect the Organic Acids sample from the first-morning midstream urine.
- Avoid apples, grapes, pears, raisins, cranberries and wine for 48 hours before urine collection.
- Unless your treating professional advises otherwise, avoid nutritional supplements for three days before sample collection so that both component tests are prepared consistently.
- Do not stop prescribed medication or medically necessary supplementation without the approval of your treating professional.
- Record recent antibiotics, medication and supplement use because these factors may influence interpretation.
- Follow the supplied instructions for blood collection, sample identification, urine storage and return transport.
Turnaround time and reporting
Because the component analyses have different processing schedules, allow approximately 10–14 working days after all suitable samples have arrived at the laboratory.
You receive laboratory reports with measured results and reference ranges for both tests, with interpretive comments where applicable. Integrated professional interpretation is recommended when results are complex or discordant.
How to interpret the results
The combined panel is most informative when the results are evaluated as patterns rather than isolated abnormalities. A functional metabolite may be influenced by several enzymes and cofactors, while a circulating vitamin concentration does not always reflect intracellular utilisation. Interpretation should therefore consider symptom pattern, diet, supplement exposure, medication, renal and hepatic function, gastrointestinal status and relevant routine laboratory findings.
This panel can support targeted nutritional and metabolic assessment, but it is not a stand-alone diagnostic test and should not be used to infer a disease or prescribe treatment from a single marker.
