Why combine these tests?
Cortisol and melatonin are complementary—but different—signals within the sleep–wake cycle. Cortisol normally follows a daytime rhythm associated with waking, activity and adaptation to demand. Melatonin should remain relatively low during daylight and rise toward biological night. Comparing both timed profiles provides more useful circadian context than examining either hormone alone when stress, energy and sleep concerns overlap.

Measures free salivary cortisol at four prescribed times during the day. The pattern helps assess the morning level, daytime progression and evening wind-down rather than relying on a single measurement.
Test 2 · Saliva
Four-Point Melatonin Profile
Measures biologically available salivary melatonin at four prescribed times across one day. The pattern assesses daytime suppression, the evening rise and the night-time level in relation to the patient’s sleep schedule.
What the combined panel can help clarify
Daytime cortisol rhythm: whether the measured cortisol pattern follows the expected progression across the collection day.
Evening wind-down context: whether late-day cortisol findings may be relevant when difficulty relaxing or falling asleep is reported.
Melatonin rhythm: whether melatonin remains suppressed during the day and shows an appropriate rise toward the patient’s biological night.
Phase and timing context: the timed pattern may help identify whether the expected melatonin rise appears earlier, later or less clearly than anticipated.
Stress–sleep relationships: both profiles can be interpreted together with sleep timing, fatigue, work schedule, light exposure, medication and lifestyle.

Sample requirements
Sample type: saliva collected at prescribed times over one day using the supplied tubes.
Cortisol profile: four timed cortisol measurements across the day.
Melatonin profile: four timed measurements spanning morning, daytime, evening and bedtime.
Collection location: non-invasive home collection following the combined WHL kit schedule.
Before collection
✓ Follow the exact collection times and storage instructions supplied with the WHL kit.
✓ Choose a representative day and record unusual sleep, travel, illness or shift-work circumstances.
✓ Report relevant medication and supplements, particularly corticosteroids, melatonin, beta-blockers and sleep medication.
✓ Follow the saliva instructions concerning food, drinks, smoking, oral hygiene and sample contamination.
Important: Do not stop prescribed medication for testing unless the prescribing healthcare professional advises you to do so.
Understanding the result
The report should be interpreted by comparing the two time-dependent curves rather than viewing individual results in isolation. Timing accuracy, usual sleep schedule, light exposure, medication, supplements, acute stress and collection-day behaviour may all influence the findings.
A four-point profile provides clinically useful circadian information but is not equivalent to a tightly controlled dim-light melatonin onset study or a formal sleep study. Professional interpretation is recommended when symptoms are persistent, severe or medically complex.
Clinical note: This panel supports biochemical and circadian assessment. It does not diagnose a primary sleep disorder, a psychiatric condition or adrenal disease, and it should not be used as a stand-alone diagnosis of “adrenal fatigue.” Results should be interpreted together with symptoms, medical history, sleep pattern, medication and other relevant investigations.
