Why combine these tests?

Blood testing measures circulating hormones and their regulatory context, including FSH, LH, SHBG, thyroid function and cardiometabolic markers. The 24-hour urine profile adds information about hormone production across a full day and how estrogens, progesterone and androgens are metabolised. Together, the tests distinguish hormone levels from downstream metabolism and provide a broader view than either assessment alone.

Test 1 · Blood

Women Essential Perimenopause

Measures FSH, LH, estradiol, progesterone, SHBG, total and free testosterone, DHEA-S, cortisol, a complete thyroid panel, HbA1c, HOMA-IR and a lipid panel.

Test 2 · 24-Hour Urine

Vital Balance Hormone Panel

Provides detailed estrogen, progesterone and androgen assessment, including hydroxy- and methoxy-estrogen metabolites, pathway ratios, pregnanediol metabolites, DHT-related metabolites and 5α-reductase activity.

What the combined panel can help clarify

Perimenopausal endocrine context: FSH, LH, estradiol and progesterone are interpreted alongside cycle history and symptoms rather than as isolated values.

Available androgens: total testosterone, free testosterone, SHBG and DHEA-S add circulating androgen context.

Estrogen metabolism: 2-OH, 4-OH and 16-OH pathways, methoxy estrogens and calculated ratios provide information about downstream estrogen handling.

Progesterone and androgen metabolites: pregnanediols, DHT-related markers, androsterone, etiocholanolone and pathway ratios extend beyond serum hormone concentrations.

Sample requirements

Women Essential: venous blood collected in the WHL-specified tubes.

Vital Balance: all urine collected over a complete 24-hour period using the supplied collection materials.

Planning: blood and urine timing may need to reflect menstrual-cycle stage, symptoms and hormone-therapy schedule.

Before collection

✓ Arrange the collection day with WHL or your practitioner when menstrual cycles are still present or irregular.

✓ Fast according to the WHL instructions because the blood panel includes HOMA-IR and metabolic markers.

✓ Record hormonal contraception, menopausal hormone therapy, corticosteroids and relevant supplements with dose and timing.

✓ Collect every urine void during the specified 24-hour interval and follow the storage and return instructions exactly.

Important: Do not stop prescribed hormones or other medication for testing unless the prescribing healthcare professional gives specific instructions.

Clinical note: Perimenopause is assessed clinically, and hormone concentrations can fluctuate substantially during this transition. This panel supports—not replaces—medical evaluation. Urinary hormone metabolites and pathway ratios do not diagnose or predict hormone-sensitive cancer. New postmenopausal bleeding, very heavy bleeding, a breast lump, severe pelvic pain or other concerning symptoms requires prompt clinical assessment.
